A Case of Extraskeletal Osteogenic Sarcome , Possibly in Myositis Ossificans

Abstract
A case of extraskeletal osteogenic sarcoma is reported in view of its rarity and of interest in its controversial aspect. The patient was a 38 years old female who had complained a hard mass in anterior portion of the left thigh with size over an adult¢¥s head.
The mass had been growing up very slowly since her age of 13, without any symptoms and functional difficulties. Bus since eight months prior to admission, she began to notice localized inflammatory reaction and generalized mild fever.
On physical examination, the mass measured 30¡¿20¡¿15 cm and the surface was smooth except several ulcerative lesions on the upper portion. The lowver portion was bony hard but the upper portion was relatively soft in consistency. On X-ray examination, this mass had no relation to femur, and linear calcification of deep muscle fascia suggested myositis ossificans. But malignant change in its upper portion of the mass was alos suspected.
The mass was removed surgically. On gross examination, it weighed 6.1 kg and was composed of two distinct patterns, bone-like lower half and soft, necrotic and hemorrhagic upper half. But there were neither distinct demarkation nor capsule.
On microscopic examination, a characteristic osteogenic sarcoma pattern was noted with highly anaplastic fibrosarcomatous cells forming bony tissue, tumor giant cells and osteoid matrix, mainly in the soft upper portion. In the lower portion, well oriented bony structures with fibrotic marrow spaces were found.
A brief review of literatures with special reference to a possibility of malignant transformation of myositis ossificans was made.
